Main question is: do you need to hold the buff for enough time to generate a grenade at levels prior to 5, or their timers stack up and youll get 1 grenade every X kills?

You seem to accumulate fractional grenades. Even one point in Grenadier will give you grenades, eventually, without pacing your kills to keep the timer going. This is an excellent skill once you get used to it: a full supply of grenades means you're not using them fast enough! If you've got a high Grenadier skill just spam the things like crazy.

Theory: The actual GpM (Grenades per Minute) may be 1.5*(Skill Level), with shown values being rounded up. Thus the actual rate of regeneration then is one grenade every [60/[1.5*(Skill Level)] seconds, which would make the left table approximately correct. If you really do regenerate the 'listed' number of grenades per minute, the right table is correct. At even skill points it doesn't matter.
